Intensive Outpatient Program (IOP) in Massachusetts

IOP carries the same clinical depth as PHP — group and individual therapy plus psychiatric services — at a lower daily intensity of about three hours a day, three to five days a week. It is designed for adults building back toward independence, whether stepping down from PHP or stepping up from weekly therapy that isn't enough.

The Intensive Outpatient Program (IOP) at Grand Rising Behavioral Health meets three to five days a week for roughly three hours a day. It carries forward everything that makes structured treatment work — consistent group and individual therapy, psychiatric services delivered inside the program, and evidence-based care — while returning a meaningful share of each day to you.

For most of our clients, IOP is where they spend the longest stretch of treatment. Some arrive by stepping down from PHP, ready for more independence but not yet for weekly appointments alone. Others step up from outpatient therapy when once-a-week sessions aren't holding against their symptoms.

That middle ground is deliberate. Recovery isn't only what happens in the therapy room — it is practicing new skills at work, at home, and in relationships, then bringing what happened back to your clinical team while it is still fresh. IOP is built around exactly that rhythm.

Who IOP Is Designed For

IOP fits adults who need consistent clinical support but are ready to carry more of daily life themselves. It may be the right level of care if you are:

  • Stepping down from PHP — you have stabilized in a full-day program and want to keep the structure while regaining independence.
  • Stepping up from weekly therapy — one session a week isn't containing your symptoms, but you don't need a full clinical day.
  • Balancing treatment with work or school — the roughly three-hour day fits alongside part-time work or a phased return to your responsibilities.
  • Managing co-occurring conditions — mental health and substance use challenges that need integrated care several days a week.
  • Rebuilding daily routines — you want frequent clinical contact while you re-establish the rhythms of everyday life that treatment ultimately has to hold up in.

What's Included in IOP

  • Group and individual therapy at a sustainable pace. The same clinical work as PHP — skills-focused groups plus one-on-one sessions — at a lower daily intensity.
  • Psychiatric services within the program. Medication management and psychiatric care continue inside IOP, coordinated with your therapists rather than handled separately.
  • The same evidence-based modalities as PHP. CBT, DBT, and trauma-informed care — nothing about the quality of your treatment changes, only its intensity.
  • A schedule built for real life. Programming fits alongside part-time work or a phased return, with evening programming available — in person at our Norwood location or via telehealth for eligible Massachusetts residents.

Conditions We Treat in IOP

Group therapy session at Grand Rising Behavioral Health

IOP treats the same range of conditions as our PHP — mental health, substance use, and co-occurring presentations — with one integrated team treating the whole picture at a lower daily intensity.

  • Anxiety — persistent worry and panic that weekly therapy alone hasn't been able to contain.
  • Depression — rebuilding energy, routine, and momentum with several clinical touchpoints each week.
  • PTSD and trauma — trauma-informed care and evidence-based therapies including EMDR.
  • Substance use and co-occurring conditions — dual-diagnosis care that addresses substance use and mental health together, which matters most in the stretch of treatment where independence is growing.

IOP Frequently Asked Questions

What is the difference between IOP and PHP?+

Intensity. PHP runs five days a week for about five hours a day — the most structured level of outpatient care we offer. IOP runs three to five days a week for about three hours a day, with the same group and individual therapy, the same psychiatric services, and the same modalities. A clinical assessment determines which level fits where you are right now, and many clients move from PHP into IOP as they progress.

How is IOP different from seeing a therapist once a week?+

Frequency and breadth. Instead of one hour a week with one clinician, IOP gives you multiple clinical days each week combining group therapy, individual sessions, and psychiatric care from a coordinated team. When symptoms are active, that difference in dose is often what makes progress hold.

Can I work or attend school during IOP?+

Yes — IOP is specifically designed to fit alongside part-time work, school, or a phased return to your responsibilities, and evening programming is available. Many clients use IOP as the bridge between full-time treatment and full-time life.

Do you take insurance for IOP?+

Grand Rising is an out-of-network provider. We work with PPO plans that include out-of-network benefits and accept private pay, and our admissions team will verify your benefits before you commit to anything. Call us today to have your coverage checked.

How do I get started with IOP?+

Call our 24/7 admissions line — available 365 days a year. We will listen to what is going on, verify your benefits, and schedule a clinical assessment to confirm whether IOP or another level of care is the right starting point.
