PHP, IOP, and OP are the three main levels of outpatient behavioral health care — the same idea (structured care while you live at home) at three different intensities. In Massachusetts, PHP is often called “Day Treatment.” This guide explains how they differ, who each is for, and how to choose. Grand Rising offers all three in Norwood, MA, with telehealth for Massachusetts residents.
All three are outpatient — you return home after each session. What changes is how many days and hours per week:
Yes. In Massachusetts, a Partial Hospitalization Program is often licensed and referred to as Day Treatment. If a provider, hospital, or insurer mentions day treatment, they’re describing PHP-level care — full days of structured treatment while you return home each evening.
Our most structured outpatient level of care. PHP blends group therapy, individual sessions, and psychiatric services into a full daily schedule — built for adults who need the daily rhythm to stabilize after a hospitalization, during a behavioral health crisis, or when session-based therapy hasn't held.
Programming designed for adults transitioning toward greater independence while still needing consistent clinical support. This is where most of our clients spend the longest stretch of treatment — stepping down from PHP or stepping up from session-based therapy that isn't enough on its own.
A short-term step-down phase after a structured program — focused on reinforcing the skills you built and handing off to your long-term community provider. It isn't an entry point and it isn't ongoing therapy. Admission to our programming goes through PHP or IOP first.

PHP is the most intensive level of outpatient care. At Grand Rising it runs 5 days a week for about 5 hours a day, combining group therapy, individual sessions, skills work (CBT, DBT), and psychiatric and medication management. It is often the right starting point when symptoms are significantly disrupting daily life, after an inpatient stay or detox, or when a lighter level has not been enough.
IOP delivers much of the same evidence-based care as PHP at a lower daily intensity — about 3 hours a day, 3–5 days a week — so you can keep working, going to school, or caring for family while still getting real structure. Many people either start at IOP or step down to it from PHP.
OP is the lightest, most flexible level — roughly one day a week, 1–3 hours. It is designed to reinforce and maintain progress, usually for adults who have completed PHP or IOP and want continued support as they return to routine.
PHP is more intensive than IOP — more days and more hours. Which one fits depends less on a label and more on what is happening right now: how much symptoms interfere with daily functioning, whether you are stepping down from a hospital or detox, and what you can sustain alongside work or family. A brief clinical assessment matches you to the right level and adjusts it as you progress.
Three simple steps from your first call to your first session — no referral needed.
Call our 24/7 admissions line or send a message. We'll ask about symptoms, safety needs, and your goals, then schedule a comprehensive assessment. Your clinician will recommend the Partial Hospitalization Program (PHP), Intensive Outpatient Program (IOP) & Outpatient Program (OP) based on functioning, risk, and availability, and will explain why.
Grand Rising Behavioral Health is out-of-network for insurance. Many clients with PPO plans use out-of-network benefits; others choose private pay. Our team will verify benefits, estimate costs and deductible implications, and provide documentation to help with reimbursement.
