Follow one client's insurance claim from her first appointment to her final statement.
Grand Rising Behavioral Health is an out-of-network provider. We accept all insurance with Out of Network benefits. We also offer self-pay options, and our patient advocates can provide more information.
Before treatment begins, our team verifies your benefits and gives you a written estimate of what you can expect to pay. Your insurance company makes the final decision on each claim, so your final amount may be different from the estimate. If it is, we will explain why.
After each visit, our billing partner submits your claim to your insurance company, so you don't have to file it yourself. Your insurance company will then send you an Explanation of Benefits (EOB). An EOB explains how your claim was processed. It is not a bill.
If you owe a balance after your insurance company processes a claim, we will send you a separate statement that shows the amount due. Some insurance companies send their payment to you instead of to us. If that happens, please contact our billing team.

For patients who are uninsured or are not using insurance
You have the right to receive a Good Faith Estimate explaining how much your health care is expected to cost.
If you do not have insurance or choose not to use insurance for your care, Grand Rising Behavioral Health will provide a written Good Faith Estimate of the expected charges for your scheduled treatment. This includes the services and related charges reasonably expected as part of your care.
Your estimate is based on the information available when it is prepared. Your treatment needs may change, and we will provide an updated estimate when required.
If you receive a bill from a provider or facility that is at least $400 more than the estimate from that provider or facility, you may dispute the bill through the federal patient-provider dispute resolution process.
Keep a copy of your Good Faith Estimate for your records.
The Good Faith Estimate rights described above apply to uninsured and self-pay patients. If you are using insurance, Grand Rising Behavioral Health collects only the deductible and coinsurance assigned to you on your insurer’s Explanation of Benefits (EOB). We do not bill you for the difference between our charges and your insurer’s allowed amount.
Your exact deductible and coinsurance responsibility depends on your policy and cannot be confirmed until your insurer processes your claims.
To request a Good Faith Estimate or ask about treatment costs, please contact our billing team.
